Christian Battle of the Bands Sign-up Form

Please fill out the Christian Battle of the Bands form and let us know the names of your members. This form will redirect you to a PayPal account for Payment fot the event. If you are under 18 you must enter a parent or guardian's name and contact info.

Please note that their is a $20 entry fee to participate.

If the members of the band are under 18 please list the primary parent/guardian's information so that we can speak with every parent
