All boxes need to be filled out.

Please fill out the Songwriter Sign-up form and let us know your name. This form will redirect you to a PayPal account for Payment fot the event. If you are under 18 you must enter a parent or guardian's name.

Please note that their is a $10 entry fee to participate.

If the members of the band are under 18 please list the primary parent/guardian's information so that we can speak with every parent
